บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาคุณภาพการดูแลแบบประคับประคองตามรูปแบบการจัดการรายกรณีในผู้ป่วยมะเร็ง

วิธีการศึกษา: การวิจัยนี้เป็นการวิจัยเชิงผลลัพธ์โดยใช้กรอบแนวคิดการประเมินคุณภาพบริการสุขภาพของโดนาบีเดียนเป็นแนวทางในการศึกษา กลุ่มตัวอย่างประกอบด้วยผู้ป่วยมะเร็ง ที่ได้รับการดูแลแบบประคับประคอง 60 คน ครอบครัวผู้ดูแล 60 คน กลุ่มบุคลากรทีมสุขภาพ 7 คน และผู้บริหารโรงพยาบาล 3 คน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและนำข้อมูล เชิงคุณภาพมาจำแนกวิเคราะห์เนื้อหา

ผลการศึกษา: ด้านโครงสร้าง พบว่าโรงพยาบาลมีนโยบายเกี่ยวกับการจัดระบบบริการสำหรับผู้ป่วยนอกและผู้ป่วยใน การจัดสรรทรัพยากรบุคคล การจัดสรรงบประมาณ คลังอุปกรณ์ที่ จำเป็นในการดูแลผู้ป่วย มีการจัดสถานที่/หน่วยบริการ และมีการเชื่อมโยงระบบบริการระหว่างโรงพยาบาลกับเครือข่ายระดับต่าง ๆ สำหรับด้านกระบวนการ พบว่ามีพยาบาลผู้จัดการรายกรณีที่มีบทบาทในการคัดเลือกผู้ป่วยเข้าไว้ในการดูแล ประเมินและระบุปัญหา ร่วมวางแผนการดูแล ประสานงานการดูแลตามแผน ประเมินและติดตามผล กำกับติดตามอย่างต่อเนื่อง และประสานงานกับเครือข่ายบริการเพื่อเชื่อมโยงข้อมูลเพื่อให้ผู้ป่วยได้รับการดูแลที่ต่อเนื่อง ส่วนด้านผลลัพธ์ พบว่าผู้ป่วยทุกรายได้รับการจัดการอาการรบกวน และได้เสียชีวิตตามความประสงค์ และร้อยละ 92.31 ได้รับการดูแลรักษาที่สอดคล้องกับการวางแผนการดูแลที่วางแผนไว้ล่วงหน้า กลุ่มผู้ป่วยและกลุ่มครอบครัวผู้ดูแลมีความพึงพอใจต่อคุณภาพการดูแลตามรูปแบบนี้โดยรวมอยู่ในระดับมาก และรูปแบบการดูแลนี้สามารถส่งเสริมให้มีการทำงานเป็นทีมสหสาขาวิชาชีพในการดูแลแบบประคับประคองอย่างมีประสิทธิภาพ

สรุป: ผลการวิจัยครั้งนี้สามารถนำไปใช้เป็นข้อมูลพื้นฐานเพื่อสนับสนุนการพัฒนาและปรับปรุงคุณภาพการดูแลแบบประคับประคองของโรงพยาบาล การศึกษาครั้งต่อไปควรทำวิจัยเพื่อพัฒนารูปแบบการพัฒนาสมรรถนะของพยาบาลผู้จัดการรายกรณีสำหรับการดูแลแบบประคับประคองและศึกษาผลของรูปแบบนั้น
