The Effectiveness of Implementing Clinical Practice Guidelines in Promoting Advanced Care Planning for Cancer Patients Receiving Palliative Care: Implementation research

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาประสิทธิผลของการใช้แนวปฏิบัติทางคลินิกในการส่งเสริมการวางแผนดูแลล่วงหน้าสำหรับผู้ป่วยโรคมะเร็งที่ได้รับการดูแลแบบประคับประคอง
วิธีการศึกษา: การวิจัยนี้เป็นวิจัยดำเนินการ (implementation research) ที่มีการนำแนวปฎิบัติทางคลินิกไปใช้ตามกรอบแนวคิดของสภาวิจัยทางการแพทย์และสุขภาพแห่งชาติ ประเทศออสเตรเลีย ประกอบด้วย 1) การจัดพิมพ์และเผยแพร่แนวปฏิบัติ 2) การใช้แนวปฏิบัติ และ
3) การประเมินผลการใช้แนวปฏิบัติ กลุ่มตัวอย่างแบ่งเป็น 2 กลุ่มคือ กลุ่มที่ 1 กลุ่มผู้ป่วยมะเร็งที่ได้รับการดูแลแบบประคับประคองแบ่งเป็นกลุ่มที่มารับการรักษาก่อนมีการใช้แนวปฏิบัติทางคลินิกในการส่งเสริมการวางแผนดูแลล่วงหน้าจำนวน 52 คน และกลุ่มที่มารับการรักษาขณะที่มีการใช้แนวปฏิบัติทางคลินิกจำนวน 33 คน และกลุ่มที่ 2 คือบุคลากรผู้ใช้แนวปฏิบัติทางคลินิกจำนวน 14 คน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา
ผลการศึกษา: 1) สัดส่วนของกลุ่มตัวอย่างที่มีการระบุความต้องการในการวางแผนดูแลล่วงหน้าระหว่างกลุ่มตัวอย่างก่อนมีการใช้แนวปฏิบัติทางคลินิกและกลุ่มตัวอย่างขณะมีการใช้แนวปฏิบัติทางคลินิก พบว่า ด้านระบุความปรารถนาของผู้ป่วยและครอบครัว ความคาดหวัง ความต้องการและทางเลือกการรักษา คิดเป็นร้อยละ 11.54 และ 81.82 ตามลำดับ ส่วนด้านแผนการดูแลเพื่อบรรเทาอาการไม่สุขสบาย คิดเป็นร้อยละ 23.08 และ 90.91 ตามลำดับ และ 2) บุคลากรผู้ใช้แนวปฏิบัติทางคลินิกในการส่งเสริมการวางแผนดูแลล่วงหน้าสำหรับผู้ป่วยโรคมะเร็งที่ได้รับการดูแลแบบประคับประคอง ส่วนใหญ่มีความพึงพอใจโดยรวมต่อการใช้แนวปฏิบัติอยู่ในระดับมากคิดเป็นร้อยละ 85.71
สรุป: ผลการวิจัยครั้งนี้แสดงให้เห็นว่าการใช้แนวปฏิบัติทางคลินิกในการส่งเสริมการวางแผนดูแลล่วงหน้าสำหรับผู้ป่วยโรคมะเร็งที่ได้รับการดูแลแบบประคับประคองช่วยส่งเสริมให้ผู้ป่วยโรคมะเร็งสามารถวางแผนดูแลล่วงหน้าและบุคลากรผู้ใช้แนวปฏิบัติมีความพึงพอต่อแนวปฏิบัติอยู่ในระดับมาก ดังนั้นจึงควรเสนอผลการวิจัยต่อผู้บริหารโรงพยาบาลเพื่อให้มีการนำแนวปฏิบัตินี้ไปใช้ในการดูแลผู้ป่วยต่อไป
