ความชุกและปัจจัยเสี่ยงของภาวะเครียดในผู้ดูแลหลักของผู้ป่วยเด็กโรคสมาธิสั้น

บทคัดย่อ
วัตถุประสงค์ เพื่อสำรวจความชุกของภาวะเครียดและปัจจัยที่เกี่ยวข้องในผู้ดูแลหลักของผู้ป่วยเด็กโรคสมาธิสั้น
วัสดุและวิธีการ เป็นการศึกษาเชิงพรรณนาชนิดภาคตัดขวางในผู้ดูแลหลักของผู้ป่วยเด็กโรคสมาธิสั้นที่มารับบริการ ณ ห้องตรวจจิตเวชเด็กและวัยรุ่น รพ.มหาราชนครเชียงใหม่ ระหว่างเดือนตุลาคม 2560 ถึงเดือนมีนาคม 2561 จำนวน 113 คน รวบรวมข้อมูล โดยใช้แบบสอบถามลักษณะของผู้ดูแลหลัก ผู้ป่วยเด็กโรคสมาธิสั้น และครอบครัว แบบวัดความเครียดสวนปรุง แบบประเมินการทำหน้าที่ของครอบครัว และแบบประเมินอาการสมาธิสั้นชนิด SNAP-IV วิเคราะห์ข้อมูลความชุกด้วยร้อยละ และใช้การวิเคราะห์ถดถอยโลจิสติคในการหาปัจจัยที่เกี่ยวข้องกับภาวะเครียด
ผล พบภาวะเครียดในผู้ดูแลหลักของผู้ป่วยเด็กโรคสมาธิสั้นร้อยละ 25 (29 คน) โดยปัจจัย ที่เพิ่มความเสี่ยงต่อการเกิดภาวะเครียดของผู้ดูแลหลัก ได้แก่ ผู้ป่วยเด็กที่มีอาการด้านขาดสมาธิรุนแรง (aOR 30.27, p=0.001, 95%CI=3.70-247.62) ผู้ป่วยเด็กเพศหญิง (aOR=7.10, p=0.005, 95%CI=1.83-27.56) และการที่ผู้ป่วยเด็กไม่มีโรคประจำตัวอื่นๆ ร่วมด้วย (aOR=3.53, p=0.034, 95%CI=1.10-11.35)
สรุป ความชุกของภาวะเครียดในผู้ดูแลหลักของผู้ป่วยเด็กโรคสมาธิสั้นเท่ากับร้อยละ 25 และสัมพันธ์กับผู้ป่วยเด็กที่มีอาการด้านขาดสมาธิผู้ป่วยเด็กเพศหญิงและผู้ป่วยเด็กที่ไม่มีโรคประจำตัวอื่นๆ ร่วมด้วย ดังนั้น ควรพิจารณาเรื่องการป้องกัน คัดกรอง และรักษาภาวะเครียดในผู้ดูแลหลักที่มีปัจจัยดังกล่าว
