บทคัดย่อ
วัตถุประสงค์: เพื่อทบทวนนิยามของการใช้ยาหลายขนาน เครื่องมือที่ใช้ประเมินความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากยา และแนวทางลดการใช้ยาที่อาจก่อให้เกิดอาการไม่พึงประสงค์รุนแรงต่อผู้ป่วยจิตเวชสูงอายุ
วัสดุและวิธีการ: ทบทวนวรรณกรรมจากเอกสารภาษาไทยและภาษาอังกฤษด้วย PubMed และ Google Scholar ในช่วงปี 2010 - 2025 ใช้คำสำคัญ ได้แก่ polypharmacy OR psychotropic polypharmacy, potentially inappropriate medication and elderly, deprescription, anticholinergic burden, drug burden index และ sedative load พบ 3,660 รายการ ได้คัดเลือกมาทบทวนวรรณกรรม 30 รายการ
ผล: เครื่องมือประเมินความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากยา ได้แก่ เครื่องมือระบุรายการยาที่ไม่เหมาะสม เช่น Beers criteria และ STOPP/START criteria เครื่องมือประเมินระดับความเสี่ยง ได้แก่ anticholinergic burden, sedative load และ drug burden index กระบวนการลดการสั่งใช้ยา ประกอบไปด้วย การประเมินรายการยา การประเมินความเสี่ยงต่ออาการไม่พึงประสงค์ การประเมินความเป็นไปได้ในการหยุดหรือลดยา การจัดลำดับความสำคัญ การดำเนินการ
และการติดตามผลหลังหยุดหรือลดยา
สรุป: การใช้ยารักษาอาการจิตเวชหลายขนาน คือ การใช้ยาจิตเวช 2 ขนานขึ้นไป เพื่อรักษาอาการเดียว ผู้ป่วยจิตเวชสูงอายุมีโอกาสได้รับยาหลายขนาน โดยเฉพาะยาที่มีฤทธิ์ต้านโคลิเนอร์จิกหรือฤทธิ์กดระบบประสาทส่วนกลาง การใช้เครื่องมือระบุรายการยาที่ไม่เหมาะสมและเครื่องมือประเมินระดับความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากยา จะลดความเสี่ยงต่ออาการไม่พึงประสงค์ที่รุนแรงจากยาได้
