การพัฒนายางกันกัดเพื่อใช้ร่วมในการรักษาด้วยไฟฟ้าในผู้ป่วยจิตเวช โรงพยาบาลสวนปรุง จังหวัดเชียงใหม่

บทคัดย่อ
การรักษาด้วยไฟฟ้า (Electroconvulsive Therapy: ECT) เป็นวิธีการรักษาที่มีประสิทธิภาพในผู้ป่วยจิตเวชที่มีอาการรุนแรง หรือไม่ตอบสนองด้วยการรักษาด้วยวิธีอื่น อย่างไรก็ตามการหดเกร็งของกล้ามเนื้อขณะทำหัตถการอาจก่อให้เกิดภาวะแทรกซ้อนในช่องปากได้ เช่น ฟันแตก ฟันเคลื่อน และแผลฉีกขาดที่ลิ้น โดยเฉพาะถ้ายางกันกัดที่ไม่เหมาะสมกับลักษณะช่องปากของผู้ป่วย
ผู้นิพนธ์จึงพัฒนายางกันกัดในการรักษาด้วยไฟฟ้า เพื่อลดภาวะแทรกซ้อนในช่องปาก บทความนี้นำเสนอแนวคิด กระบวนการพัฒนา ตั้งแต่ปี พ.ศ. 2560 ถึงปัจจุบัน ผลลัพธ์ของยางกันกัดสวนปรุงโมเดล 1 ซึ่งออกแบบให้เหมาะสมกับการรักษาด้วยไฟฟ้าทั้งแบบใช้ยานำสลบและไม่ใช้ยานำสลบ และโมเดล 2 ซึ่งเน้นแก้ภาวะแทรกซ้อนที่ยังคงเป็นปัญหาคือแผลกัดฉีกที่ลิ้น ผลการติดตามพบว่า หลังใช้ยางกันกัด อุบัติการณ์และความรุนแรงของภาวะแทรกซ้อนในช่องปากลดลง โดยเฉพาะยางกันกัดสวนปรุงโมเดล 2 ไม่พบภาวะแทรกซ้อนเลย นอกจากนี้ ต้นทุนการผลิตต่ำกว่า ช่วยลดค่าใช้จ่ายเมื่อเทียบกับการสั่งซื้อจากภายนอก ผู้ใช้งานมีความพึงพอใจในระดับสูง เหมาะสมต่อการประยุกต์ใช้ในบริบทของโรงพยาบาลสวนปรุง และควรมีการขยายผลการใช้งานยางกันกัดที่พัฒนาขึ้นไปยังหน่วยงานจิตเวชอื่นๆ พร้อมทั้งศึกษาผลลัพธ์ในระยะยาว
