บทคัดย่อ
วัตถุประสงค์ เพื่อหาปัจจัยที่มีผลต่อความเสี่ยงในการฆ่าตัวตายของกลุ่มผู้ป่วยโรคเรื้อรังที่มารับบริการผู้ป่วยนอกโรงพยาบาลแม่ใจ
วัสดุและวิธีการ เป็นการศึกษาเชิงพรรณนา รูปแบบ Unmatched Case control study กลุ่มตัวอย่างคือผู้ป่วยโรคเรื้อรัง จำนวน 525 คน แบ่งเป็น กลุ่มศึกษา(case=75) คือกลุ่มผู้ป่วยที่มีความเสี่ยงฆ่าตัวตาย และกลุ่มควบคุม (control n=450) คือกลุ่มผู้ป่วยที่ไม่มีความเสี่ยงในการฆ่าตัวตายคัดเลือกด้วยวิธีสุ่มอย่างง่าย วิเคราะห์อธิบายข้อมูลโดยใช้สถิติเชิงพรรณนา เพื่อหาความสัมพันธ์ระหว่างปัจจัยแบบตัวแปรเดี่ยว ใช้สถิติ Chi-square test และสถิติการวิเคราะห์ความสัมพันธ์ระหว่างปัจจัยแบบหลายตัวแปร (Multiple logistic regression) นำเสนอค่า Adjusted odds ratio (ORadj)
ผล ปัจจัยที่มีผลต่อความเสี่ยงในการฆ่าตัวตาย ได้แก่อายุ 65 ปีขี้นไป (ORadj =2.85 ;95% CI =1.23-6.40) มีความพิการ (ORadj =1.92; 95% CI = 1.02-4.09) มีระยะที่ป่วยเป็นโรคเรื้อรังมากกว่า 10 ปีขึ้นไป (ORadj =2.52; 95% = CI 1.33-4.75), และมี หรือเคยมีภาวะซึมเศร้า (ORadj = 16.47; 95% CI = 8.87-30.57)
สรุป: ปัจจัยที่มีผลต่อความเสี่ยงในการฆ่าตัวตายในผู้ป่วยกลุ่มโรคเรื้อรังได้แก่อายุ 65 ปีขึ้นไป มีความพิการ ระยะเวลาที่ป่วยด้วยโรคเรื้อรังมากกว่า 10 ปี และมีประวัติโรคซึมเศร้าหรือมีภาวะซึมเศร้า
