บทคัดย่อ

ปัญหาสำคัญที่พบภายหลังการผ่าตัดเปลี่ยนข้อเข่าเทียม(total knee arthroplasty) คือ การเสียเลือดหลังการผ่าตัดปริมาณมาก  จึงได้มีการนำ tranexamic acid มาใช้ในระหว่างการผ่าตัดเพื่อลดปริมาณการเสียเลือด โดยนำมาใช้ในรูปแบบทั้งการฉีดเข้ากล้ามเนื้อ ฉีดเข้าเส้นเลือด และฉีดเข้าข้อเข่าโดยตรง แต่อย่างไรก็ตามยังไม่มีการนำ tranexamic acid มาใช้ฉีดเนื้อเยื่อรอบเข่าในระหว่างการผ่าตัด การวิจัยนี้จึงมีจุดมุ่งหมายเพื่อศึกษาถึงผลการฉีด tranexamic acid เข้าเนื้อเยื่อรอบเข่าในระหว่างการผ่าตัดว่าสามารถลดปริมาณเลือดออกภายหลังการผ่าตัด total knee arthroplastyได้หรือไม่โดยทำการศึกษาในผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นข้อเข่าเสื่อมทั้งหมด 30  คนมารับการรักษาโดยการเปลี่ยนข้อเข่าเทียม โดยแบ่งผู้ป่วยเป็นทั้งหมด 2 กลุ่ม กลุ่มละ 15 คน กลุ่มทดลอง (กลุ่ม AX ได้รับการฉีดยา tranexamic acid ปริมาณ 10 mg/ kg เข้าเนื้อเยื่อรอบเข่ากลุ่มควบคุม (กลุ่ม BX ได้รับการฉีด normal saline 20 ml แทน จากนั้นเก็บข้อมูลปริมาณเลือดออกภายหลังการผ่าตัดทุก 8 ชั่วโมงจนครบ 48 ชั่วโมง, ตรวจค่าความเข้มข้นของเลือด(hematocrit, Hct)หลังผ่าตัดตั้งแต่วันที่ 1-5 และมีการเก็บข้อมูลปริมาณการให้เลือดของทั้ง 2 กลุ่มพบว่ากลุ่มได้ที่ได้รับการฉีด tranexamic acid เข้าเนื้อเยื่อรอบเข่า สามารถลดปริมาณเลือดออกรวมภายหลังการผ่าตัดอย่างมีนัยสำคัญ (p = 0.01) โดยกลุ่มที่ได้รับการฉีดยา tranexamic acid มีปริมาณเลือดออกเฉลี่ย 440 ± 144.75 ml กลุ่มที่ได้รับการฉีด normal saline มีปริมาณเลือดออกเฉลี่ย 638 ± 235.44 ml ซึ่งเมื่อเปรียบเทียบในแต่ละช่วงระยะเวลาจะพบว่าการฉีด tranexemic acid เข้าในเนื้อเยื่อรอบข้อสามารถลดการเสียเลือด ได้อย่างมีนัยสำคัญทางสถิติในเฉพาะช่วงระยะเวลา 0-8 และ 8-16 ชั่วโมงภายหลังการผ่าตัด และ ผล Hct ภายหลังการผ่าตัดของทั้ง 2 กลุ่มแตกต่างกันอย่างมีนัยสำคัญทางสถิติ เพียงเฉพาะวันแรกหลังการผ่าตัดเท่านั้น (p=0.048)  อย่างไรก็ตามปริมาณการให้เลือดของทั้ง 2 กลุ่มนั้นไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.426) การฉีดยา tranexamic acid เข้าเนื้อเยื่อรอบเข่าระหว่างการผ่าตัดเปลี่ยนข้อเข่าเทียมนั้นสามารถลดปริมาณเลือดออกภายหลังการผ่าตัดได้ เมื่อเปรียบเทียบกับการฉีด normal saline โดยที่ไม่มีผลภาวะแทรกซ้อนเช่น deep  vein thrombosis, pulmonary embolism อย่างไรก็ตามการฉีดยา tranexamic acid ไม่ได้ช่วยลดปริมาณการให้เลือดภายหลังการผ่าตัด

Abstract

Tranexamic acid is used to reduce postoperative blood loss after total knee arthroplasty (TKA). There are several methods to apply tranexamic acid such as intravenous, intramuscular, and intra-articular injection. To our knowledge, no previous study reported periarticular soft tissue injection of tranexemic acid can be used to reduce blood loss in total knee arthroplasty. To study if periarticular soft tissue injection of tranexamic acid can reduce postoperative blood loss and blood transfusion requirement after total knee arthroplasty. We have enrolled 30 patients with primary osteoarthritis undergoing a unilateral cemented total knee arthroplasty in prospective, randomize study. Patients were randomized into two groups: the study group (group A) received tranexamic acid periarticular soft tissue injection (10 mg/kg) while the control group (group B) received normal saline 20 ml injection. We have measured volume of drained blood 48 hours postoperatively, hematocrit on postoperative 1st – 5th day, and amount of blood transfused. Mean (± SD) of postoperative bleeding was significantly lower in the group receiving tranexamic acid (440 ± 144.75 ml) when compare with control subjects in group B (638 ± 235.44 ml). We found a significant difference of hematocrit on postoperative in 2 groups only on the first day (p=0.048).
The amount of blood transfused was not different between 2 groups. Periarticular soft tissue injection of tranexamic acid can reduce postoperative blood loss after total knee arthroplasty. However, tranexamic acid did not reduce blood transfusion requirement. We did not observe any complication of periarticular injection of tranexamic acid from this study.
