The use of total contact cast in treatment of chronic diabetic foot ulcers

Abstract

Chronic diabetic foot ulcers are a major complication in diabetic patients. Reducing loading pressure on the foot is the most important factor for the healing of neuropathic ulcers.  This off-loading technique can be effectively accomplished by using the total contact cast (TCC) for a limited duration.  However, TCC has not been widely utilized due to lack of knowledge, experience and expertise.  In addition, many patients are reluctant to use TCC owing to fear of wound infection and cosmetic cencerns.  This article provides a comprehensive review and describes the author’s experience with the successful use of the TCC for treatment of chronic diabetic foot ulcers.

การรักษาแผลเรื้อรังในผู้ป่วยเท้าเบาหวาน โดยการใช้เฝือกแบบสัมผัสทุกสัดส่วน

แผลเรื้อรังที่เท้าเป็นภาวะแทรกซ้อนที่สำคัญในผู้ป่วยเบาหวาน การลดแรงกดที่บริเวณฝ่าเท้าเป็นวิธีที่จะรักษาแผลที่ เกิดจากปลายประสาทเสื่อมที่ดีโดยเฉพาะวิธีการการลดแรงกดทับด้วยการใช้เฝือกแบบสัมผัสทุกสัดส่วนที่นำมารักษาแผลกลุ่มนี้ ได้ดีซึ่งใช้ระยะเวลาไม่นานและมีประสิทธิภาพสูง แต่ยังไม่เป็นที่แพร่หลายเนื่องจากพบว่ายังขาดความรู้และความชำนาญ ในการทำ ผู้ป่วยส่วนใหญ่ที่พบมักจะกังวลใจ ไม่มั่นใจ กลัวแผลติดเชื้อ กลัวภาพลักษณ์ที่ตนเองต้องใส่เฝือก ดังนั้น บทความ วิชาการนี้ได้รวบรวมข้อมูลและผลงานวิจัย รวมทั้งประสบการณ์ในการรักษาผู้ป่วยเบาหวานที่มีแผลเรื้อรังที่เท้าและได้รับการ รักษาด้วยวิธีใช้เฝือกแบบสัมผัสทุกสัดส่วน เพื่อให้ผู้อ่านได้มีความรู้ความเข้าใจเกี่ยวกับวิธีการรักษาด้วยวิธีนี้ให้มากยิ่งขึ้น
