Abstract
This participatory action research (PAR) aimed to study the elderly health care with community participation. A total of 53 volunteers (age >60 years old) were recruited by purposive sampling from two communities located around Phranakhon Rajabhat University. The data was collected by using interviewing form, Barthel activities of daily living (ADL), group discussion, and group meeting. Then, it was analyzed by descriptive statistics and content analysis. The methodology comprised of 4 processes; 1) finding and using social capital, 2) finding health problem, 3) activity design, and 4) agreement development in community.

A community with robust, dedicated leaders and members always tends to obtain the efficiency of personal capital. Besides, the selfreliance of the community by revolving fund would enable more sustainable elderly health care and provide the elderly a chance to participate in designing activities consistent to their health issues by using group process in developing agreements. Consequently, there are elderly aggregation to exercise for better health, monthly activities can be organized, some
