Abstract
Acute ischemic stroke is a medical emergency that requires prompt treatment to prevent complications and reduce mortality. This case-control study aimed to identify factors associated with timely hospital arrival (within 4.5 hours or 270 minutes after symptom onset) among patients receiving care at Kong Krailat Hospital, Sukhothai Province. A total of 80 participants were included, with 40 patients in the timely arrival group and 40 in the delayed group. Data were collected using structured checklists and questionnaires assessing stroke knowledge, recognition of warning signs, perceived severity, health literacy, and stroke-preventive behaviors based on the model from the Ministry of Public Health. Data were analyzed using multivariable analysis and logistic regression.
The results identified six significant factors associated with timely arrival : higher educational level, distance to hospital less than or equal to 15 kilometers, shorter time from symptom recognition to hospital arrival, better general knowledge of stroke, accurate recognition of warning signs and greater perception of stroke severity . These findings highlight the importance of stroke-related knowledge and behavior in reducing pre-hospital delay. Public health strategies should prioritize community education on stroke symptoms and the urgency of immediate medical attention to improve treatment outcomes.
