บทนำ : โรคหัวใจล้มเหลวเป็นภาวะเรื้อรังที่ส่งผลกระทบอย่างรุนแรงต่อคุณภาพชีวิตของผู้ป่วย ทั้งในด้านร่างกาย จิตใจ และสังคม โดยเฉพาะช่วงหลังออกจากโรงพยาบาล ผู้ป่วยจำนวนมากเผชิญความยากลำบากในการปรับตัวและการดำเนินชีวิตประจำวันอย่างมีประสิทธิภาพ
วัตถุประสงค์ : การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาระดับคุณภาพชีวิต และปัจจัยที่สัมพันธ์กับคุณภาพชีวิตของผู้ป่วยโรคหัวใจล้มเหลวหลังการนอนโรงพยาบาล ของโรงพยาบาลท่าศาลา จังหวัดนครศรีธรรมราช
วัสดุและวิธีการศึกษา : การวิจัยเชิงพรรณนาแบบภาคตัดขวาง กลุ่มตัวอย่างคือผู้ป่วยโรคหัวใจล้มเหลวที่มาติดตามรักษาในคลินิกโรคหัวใจล้มเหลว โรงพยาบาลท่าศาลา จำนวน 145 ราย เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลทั่วไป ข้อมูลทางคลินิก และแบบประเมินคุณภาพชีวิต WHOQOL-BREF-THAI วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา การทดสอบความสัมพันธ์ด้วย Chi-square test และ Independent t-test และวิเคราะห์พหุคูณด้วย Multiple Linear Regression
ผลการศึกษา : พบว่าผู้ป่วยส่วนใหญ่มีคุณภาพชีวิตระดับปานกลาง (ร้อยละ 78.62) รองลงมาคือระดับดี (ร้อยละ 11.72) โดยมีคะแนนเฉลี่ยรวม 74.54 ± 11.40 ปัจจัยที่มีความสัมพันธ์เชิงบวกกับคุณภาพชีวิตอย่างมีนัยสำคัญทางสถิติ ได้แก่ การได้รับยา Atorvastatin (β = 1.55, p = 0.007) การใช้อินซูลิน (β = 1.05, p = 0.023) การกลับไปทำงานหลังการรักษา (β = 0.90, p = 0.005) การเดินทางมาโรงพยาบาลได้ด้วยตนเอง (β= 0.85, p = 0.004) ระดับการศึกษาสูงกว่าประถมศึกษา (β = 0.70, p = 0.005) และส่วนสูง (β= 0.05, p = 0.009) ขณะที่อายุที่มากขึ้น (β = −0.04, p = 0.027) และประวัติโรคหลอดเลือดสมองเดิม (β = −1.25, p = 0.023) มีความสัมพันธ์เชิงลบกับคุณภาพชีวิต แบบจำลองสามารถอธิบายความแปรปรวนของคุณภาพชีวิตได้ร้อยละ 42 (Adjusted R² = 0.42, p < 0.001)
สรุป : คุณภาพชีวิตของผู้ป่วยโรคหัวใจล้มเหลวได้รับอิทธิพลจากหลายปัจจัย ทั้งด้านคลินิก สมรรถภาพทางกาย และเศรษฐสังคม การดูแลผู้ป่วยควรเน้นแนวทางแบบองค์รวม (Holistic care) ที่ผสมผสานการรักษาทางการแพทย์ การฟื้นฟูสมรรถภาพทางกาย และการสนับสนุนทางสังคม เพื่อยกระดับคุณภาพชีวิตของผู้ป่วยหัวใจล้มเหลวอย่างยั่งยืน
