บทคัดย่อ
ภาวะไตอักเสบ membranoproliferative glomerulonephritis เป็นลักษณะทางพยาธิวิทยาที่พบได้ในโรคเชิงระบบหลายโรค ได้แก่ โรคเอสแอลอี โรคข้ออักเสบรูมาตอยด์ โรคโซเกร็น ภาวะที่เซลล์พลาสมาผลิตอิมมูโนโกลบูลินมากผิดปกติ เป็นต้น อีกสาเหตุหนึ่งที่มีความสำคัญแต่พบได้ไม่บ่อยคือ การอักเสบของโกลเมอลูรัสอันเป็นผลจากการทำงานที่ผิดปกติของคอมพลีเมนท์ชนิด C3 โดยเรียกภาวะนี้ว่า C3 glomerulonephritis การวินิจฉัยภาวะ C3 glomerulonephritis ต้องอาศัยการตรวจทางพยาธิวิทยาของชิ้นเนื้อไตในผู้ป่วยที่มีภาวะไตอักเสบโดยไม่ทราบสาเหตุชัดเจน การวินิจฉัยที่ถูกต้องและรวดเร็ว สามารถนำไปสู่การรักษาด้วยยากดภูมิได้อย่างทันท่วงที ซึ่งอาจส่งผลให้ผลลัพธ์ทางไตดีขึ้น บทความนี้เป็นรายงานผู้ป่วยชายอายุ 73 ปี มีอาการเป็นผื่นที่ขาทั้ง 2 ข้าง ร่วมกับมีภาวะไตวายเฉียบพลัน และมีโปรตีนและเม็ดเลือดแดงรั่วในปัสสาวะ ผู้ป่วยได้รับการเจาะชิ้นเนื้อไตซึ่งพบว่าผลตรวจเข้าได้กับภาวะ
C3 glomerulonephritis การตรวจเพิ่มเติมโดย serum immunofixation ไม่พบว่ามีภาวะ monoclonal gammopathy ร่วมด้วย ซึ่งภาวะ monoclonal gammopathy นี้มักพบร่วมกับภาวะ C3 glomerulonephritis ในผู้ป่วยที่มีอายุ 50 ปีขึ้นไป
