ภาวะซึมเศร้าในโรคไตเรื้อรัง

บทคัดย่อ
ภาวะซึมเศร้าเป็นภาวะที่พบได้บ่อยในผู้ป่วยโรคไตเรื้อรัง และผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการบำบัดทดแทนไต โดยภาวะซึมเศร้ามีความสัมพันธ์กับคุณภาพชีวิตที่ลดลงและอัตราการเสียชีวิต การวินิจฉัยภาวะซึมเศร้าอาศัยการใช้แบบสอบถามชนิดต่าง ๆ เพื่อประเมินอาการของโรคเบื้องต้น และการยืนยันวินิจฉัยโรคอาศัยการวินิจฉัยโดยแพทย์หรือจิตแพทย์เป็นสำคัญ ปัจจัยเสี่ยงต่อการเกิดภาวะซึมเศร้าในผู้ป่วยโรคไตเรื้อรังมีความหลากหลาย กลไกการเกิดภาวะซึมเศร้าในผู้ป่วยโรคไตเรื้อรังมี 2 แบบ คือ ภาวะซึมเศร้าปฐมภูมิและภาวะซึมเศร้าทุติยภูมิ โดยภาวะซึมเศร้าปฐมภูมิเป็นภาวะที่ไม่เกี่ยวข้องกับความเจ็บป่วยทางร่างกาย ส่วนภาวะซึมเศร้าทุติยภูมิมีความสัมพันธ์กับความเจ็บป่วยทางกายเรื้อรัง ชนิดและขนาดของยาที่ใช้รักษาภาวะซึมเศร้าในผู้ป่วยไตเรื้อรังมีความหลากหลาย และยังไม่มีข้อสรุปยืนยันถึงประสิทธิภาพชัดเจนของยาต้านเศร้าเหล่านั้น ในอนาคตมีความจำเป็นที่จะต้องมีการศึกษาใหม่ ๆ ที่ช่วยอธิบายกลไกการเกิดภาวะซึมเศร้าในผู้ป่วยโรคไตเรื้อรัง รวมถึงการป้องกันและการรักษาที่มีประสิทธิภาพ
