บทคัดย่อ
ผู้ป่วยหญิง อายุ 67 ปี เป็นเบาหวาน มีอาการไข้ ปัสสาวะไม่คล่อง ปัสสาวะไม่สุด ปัสสาวะขุ่นและแสบขัด ปวดบั้นเอวทั้ง 2 ข้าง ช่วงแรกปัสสาวะเท่าเดิม ไม่เหนื่อย ไม่มีขาบวม ได้รับการวินิจฉัยว่าเป็น acute pyelonephritis ได้รับยาปฏิชีวนะ ต่อมาผู้ป่วยมีอาการปวดหลังด้านซ้ายมากขึ้น และปัสสาวะไม่ออก BUN/Creatinine เพิ่มสูงขึ้น computed tomography ของ KUB system และ retrograde pyelogram มีลักษณะเข้าได้กับ acute papillary necrosis
