The Effectiveness of the Project Non – communicable Diseases Management of the Office of Disease Prevention and Control 3, Nakhon Sawan Province.

Abstract
The evaluation of the effectiveness of the Non-Communicable Disease Management System have to 2 objectives the first to evaluate the effectiveness of the project, the second to evaluated impact of project. The model of evaluation using by Michael Scriven Model and Kirkpatrick Evaluation Model. The population and sample of study were the trainee and certified of NCD clinicians. The results show that were indicated that all activities are progressing according to the plan and achievement the intended purpose have effective all activities. There are so many outputs that the plan less budget spending and it can be completed before the scheduled time. The operational of results it was found that the service recipients were satisfied with good performance. The satisfaction relationship was based on communication with the evaluators (r = .867), the empowerment factor (r = .862) and the evaluator (r = .860) statistically significant at p <0.01. Moreover, the incidence of diabetes mellitus and new patient high blood pressure have dropped to only 0.82% and 1.03% from the target of not more than 5% and 10%, respectively. The recommendations from the evaluation of this project should be continue because the results of surveillance operations prevent disease control contact by adjusting to the non-overlapping activity with the Office of Communicable Disease and to integrate multiple activities should have the activities are in one project because of it can link the results of activities into the chain of success of the project well.
