Nursing Care of Ischemic stroke Patient : A case study

Abstract
Ischemic stroke mostly caused by the malfunction of blood circulation to the brain, they are the serious problems that affect patients in many aspects and can even lead to death. Those who survive from the diseases are usually disabled or deformed with affect to their lives and their families permanently. This report was to study the complete cares of the patients from the onset of disease, rehabilitation period until they can be discharged from the hospital. Patient and their families were trained with the proper cares at home and introduced to those who can provide additional help so that they have continuity of cares and avoided the recurrence of the diseases. Being hospitalized can save the lives of the critically conditioned patient and discharge planning with the patient’s family is necessary for self-care.
