Abstract

ABSTRACT

Aim: Investigate the effectiveness of implementing the psychiatric family caregiving program for patients with schizophrenia at Pangsilathong Hospital, Kamphaeng phet province.

Methods: The samples were selected by purposive sampling and included ten caregivers, ten patients with schizophrenia, and six nurses at the outpatient department of Pangsilathong Hospital from August to November 2015. The instruments consisted of 1) personal identification form, 2) the psychiatric family caregiving program, 3) the psychiatric family caregiving skills scale, 4) assessment of medication adherence behaviors scale, 5) the satisfaction questionnaire of caregivers after the program’s completion and 6) the opinion questionnaire toward the program. Data were analyzed by descriptive statistics.

Results: After attending the Psychiatric Family Caregiving Program, the caregiving skill of caregivers increased, which led them to provide comprehensive care especially duties related to medication management. This program led to medication adherence on a high level of all patients with schizophrenia. After completing the program, all caregivers  expressed high satisfaction with the program. Also, all outpatient department nurses were satisfied with implementing this program.

Conclusion: The results indicate the effectiveness of implementing the Psychiatric Family Caregiving Program for persons with schizophrenia.

บทคัดย่อ

วัตถุประสงค์ เพื่อศึกษาประสิทธิผลของการใช้โปรแกรมการดูแลผู้ป่วยจิตเวชโดยครอบครัวสำหรับผู้ป่วยโรคจิตเภทโรงพยาบาลปางศิลาทอง จังหวัดกำแพงเพชร

วิธีการ คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด ประกอบด้วย ผู้ดูแลผู้ป่วยโรคจิตเภท 10 ราย ผู้ป่วยโรคจิตเภท 10 ราย พยาบาลที่ปฏิบัติงานในแผนกผู้ป่วยนอก โรงพยาบาลปางศิลาทอง จำนวน 6 ราย ระหว่างเดือนสิงหาคม-พฤศจิกายน พ.ศ. 2558 เครื่องมือประกอบด้วย 1) แบบสอบถามข้อมูลส่วนบุคคล 2) โปรแกรมการดูแลผู้ป่วยจิตเวชโดยครอบครัว 3) แบบวัดทักษะการดูแลผู้ป่วยจิตเวชที่บ้าน 4) แบบประเมินพฤติกรรมการใช้ยาตามเกณฑ์การรักษา 5) แบบสอบถามความพึงพอใจของญาติผู้ดูแลหลังได้รับโปรแกรม และ 6) แบบสอบถามความคิดเห็นของบุคลากรทางการพยาบาลต่อการใช้โปรแกรม วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา

ผลการศึกษา หลังจากเข้าร่วมโปรแกรมการดูแลผู้ป่วยจิตเวชโดยครอบครัว ผู้ดูแลมีทักษะในการดูแลเพิ่มขึ้น ทำให้ดูแลผู้ป่วยโรคจิตเภทได้อย่างครอบคลุม โดยเฉพาะในด้านการบริหารยา ส่งผลให้ผู้ป่วยโรคจิตเภทมีความร่วมมือในการรักษาด้วยยาอยู่ในระดับสูงขึ้นทั้งหมด ผู้ดูแลทั้งหมดมีความพึงพอใจหลังได้รับโปรแกรมอยู่ในระดับสูง และพยาบาลที่ปฏิบัติงานในแผนกผู้ป่วยนอกทั้งหมดเห็นด้วยกับการใช้โปรแกรมดังกล่าว

สรุป จากผลการศึกษานี้ แสดงให้เห็นถึงประสิทธิผลของโปรแกรมการดูแลผู้ป่วยจิตเวชโดยครอบครัวสำหรับผู้ป่วยโรคจิตเภท
