Association between laboratory parameters for monitoring and the kidney complication in diabetic patients from Nakhonphanom Hospital

Abstract

Abstract

        Objective :  The study aimed to investigate the relationship between laboratory parameters results and kidney function in diabetic patients who were monitored at Nakhon Phanom Hospital, Nakhon Phanom Province

         Materials and methods :  This is retrospective study, the physical examination and laboratory informations were collected from 1459 diabetes patients who visited Nakhonphanom hospital from 1st   January  2021, to 31th December 2023 and aged > 18 years old

        Results. 1459 patients  with the majority being female (67.6%). The median duration of diabetes in the study sample was 7 years. The incidence of complications in the first, second, and third years is 36.2%, 41.4%, and 44.8% respectively. Over time, it was found that the estimated glomerular filtration rate (eGFR) significantly decreased (p < 0.0001) in diabetic patients. However, monitoring HbA1c levels once a year versus multiple times per year showed no significant relationship with kidney complications in diabetic patients (p = 0.104). Laboratory test results correlated significantly with creatinine levels in diabetic patients, with factors such as male gender, age, duration of diabetes, fasting plasma glucose (FBS), and HbA1c showing statistical significance (p-value < 0.05). Furthermore, certain laboratory test results were significantly associated with eGFR values in diabetic patients, with age and duration of diabetes being statistically significant (p-value < 0.05). Additionally, laboratory test results were significantly related to microalbumin levels in diabetic patients, with male gender and HbA1c being statistically significant (p-value < 0.05). A predicted HbA1c formula was calculated as 4.499 + (0.025 × FBS) for patients who could not undergo the standard HbA1c measurement method, and it was found to be significantly correlated with HbA1c measured by Point-of-Care Testing (POCT). The Hemoglobin Glycated Index (HGI) showed a very low correlation with kidney function.

        Conclusion: This study provides insights for predicting kidney complications in diabetic patients based on important factors and analytical limitations of lab tests.
