Evaluation of the outpatient prescriptions (Rx OPD) Program to Prevent Medication Errors outpatient dispensing in Nakhon Phanom Hospital

Abstract

Abstract: Patients who receive services at Nakhon Phanom Hospital which sometimes do not bring medicine with them cause not know which drug is being taken thus making the treatment lack of continuity The Pharmacy Department has developed a computer program system called Rx OPD to create a system for coordinating outpatient drug lists. Reduce drug prescription errors during the course of treatment and to ensure that patients receive the medication they deserve on an ongoing basis.Objective: To reduce medication errors Using the Rx OPD program, measuring results from medication errors and medication reconcile errors (Med reconcile errors).Methods: This research and development consists of 3 parts: 1. Study the nature and size of the problem of incorrect and appropriate drug prescribing for patients. In Nakhon Phanom Hospital By collecting retrospective data from the drug error database. Between January-September 2022  2. Develop the Rx OPD program to prevent medication list synchronization problems and implement it in September 2022 3. Study the results of using the Rx OPD program to prevent problems with medication list coordination errors. It is the number of times (events) calculated as a percentage. Evaluated before starting to use the program from January - September 2022 and after using the program from October 2022 - April 2023

Result: From the review of medication errors, it was found that the first priority was not the drug that the patient was taking. representing 47.14 percent. The outpatient dispensary department assessed from the drug error, that types of medication errors before and after the program. It was found that the drug discrepancy found was not the drug that the patient received. representing 47.14 and 35.15 percent, which decreased significantly (P<0.001).

Conclusion: The Rx OPD program was established for pharmacists to consider following up on previous medication history and can coordinate the medication list of patients receiving services. Able to prevent or correct medication dispensing errors that may occur to patients and make the treatment system in patients more effective
