Comparative the (Non-invasive positive pressure ventilator, NIPPV) nasal prongs lubricating between using 0.9% Normal Saline and 0.02% TA Cream to lessen nasal bleeding of newborn baby patients in Nakhon Phanom Hospital.

Abstract

Objectives: The main purpose was to compare the early time and severity of nasal bleeding on the NIPPV nasal prongs lubricating between 0.9% Normal Saline and 0.02% TA Cream.  

 Materials and method: This study is the prospective Quasi-experimental study in newborn baby patients with the NIPPV nasal prongs lubricating. The study was implemented between   

November 2016 and April 2018 in Department of Pediatric Intensive Care Unit at Nakhon Phanom hospital. The newborn baby patients were 46 cases. This study was to compare the early time and severity of nasal bleeding before and while using the NIPPV nasal prongs lubricating between 0.9% Normal Saline and 0.02% TA Cream. After consent was given, patients were randomized allocation in each group by block of four method to 23 cases. Each group of the groups were observed the early time and severity of nasal bleeding. The result was analyzed with t-test and exact probability test.  

Results: The results illustrated that each group of the group had similarity of the characteristic type. The incident of nasal bleeding before and while using the NIPPV nasal prongs lubricating was observed. 0.9% Normal Saline (13%) on nasal prongs lubricating presented the severity of nasal bleeding with mild that is significantly difference of using 0.02% TA Cream (43.4%) at p-value 0.022. The early time and severity of nasal bleeding in 0.9% Normal Saline was 27.5 hours, and 0.02% TA Cream was 40.1 hours that insignificantly difference at p-value 0.176 and 0.862 respectively.

Conclusion: The incident of nasal bleeding before and while using the NIPPV nasal prongs lubricating with 0.9% Normal Saline was mild nasal bleeding less than 0.02% TA Cream. When consider a trend of early nasal bleeding time in 0.9% Normal Saline lubricating toward more than 0.02% TA Cream. We recommended that should be use the NIPPV nasal prongs lubricating with 0.9% Normal Saline in early period. Be caution in the long term using more than 28 hours. We suggest to switch using in another non-invasive type
