Seroma reduction after modified radical mastectomy in breast cancer by flap fixation technique combined with close suction drainage, Nakhonphanom hospital

Objective: To study seroma formation after modified radical mastectomy in breast cancer between flap fixation technique combined with close suction drainage and only close suction drainage.
Materials and methods: This study was efficacy research retrospective-Cohort study in 76 female breast cancer patients who underwent modified radical mastectomy from single surgeon in Nakhonphanom hospital between June 2016 - December 2019. Data were collected from medical record forms and all patients were divided into 2 groups, equally. General data were analyzed by mean and standard deviation. Data of seroma formation, days of drain placing and cost of hospital stay were analyzed by t-test and Fisher’s extract test.
Results: General clinical description of all patients were not different. Flap fixation technique combined with close suction drainage group had no seroma formation while close suction drainage group had 15 cases of seroma formation (39.5%), statistically significant difference (p-value < 0.001). Average days of drain placing were 8.3 days (SD 1.5) in the first group and 11.7 days (SD 2.2) in the second group, statistically significant difference (p-value < 0.001). Average cost of hospital stay in first group was 28,525.5 baht (SD 3,156.1) and the second group was 30,711.4 baht (SD 3,501.3), statistically significant difference (p-value=0.006).
Conclusion: Flap fixation technique combined with close suction drainage can reduce seroma formation, day of drain placing and cost of hospital stay, statistically significant difference. Flap fixation technique combined close suction drainage should be performed to reduce seroma formation after modified radical mastectomy.
