The development of diabetic emergency risk assessment for diabetic older adults in community

Abstract

Diabetic emergency is a relative frequent event in diabetic older adults that also impacts on health. Risk factor determination in diabetic older adults can be used as a screening tool and surveillance to decreasing severity on health. This cross sectional study aimed to develop the diabetic emergency risk assessment for diabetic older adults in community. The sample were two hundred and two diabetic older adults. The diabetic emergency risk form for diabetic older adults is the main instrument for collecting data. Data were analyzed by logistic regression. Additional analyzed to find sensitivity, specificity, positive predictive value, negative predictive value, and area under curve.

                The results showed that risk factors associated with the diabetic emergency risk assessment including swallowing and chewing problem, infection, diet control and food and water consumption. By integrating risk factors into the screening tool, the cut­off point for diabetic emergency risk prediction is equally to 2.  The Sensitivity was 47.10%, the Specificity was 91.90%, the Positive Predictive Value was 34.78%, the Negative Predictive Value was 94.99% and Area Under the Curve of the ROC was 75%. The study indicates that the diabetic emergency risk assessment tool for diabetic older adults should be used as a primary screening by village health volunteers to assess diabetic emergency risk. Diabetic older adults with a score ranging from 2 to 8 points should be monitored for diabetic emergency risk and referring who are at risk for further appropriate care and treatment.
