การควบคุมการแพร่ระบาดโรคติดเชื้อไวรัสโคโรนา 2019 (COVID-19) ของโรงพยาบาลนพรัตนราชธานีในรูปแบบ Bubble and Seal

การระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 (COVID-19) เริ่มต้นที่เมืองอู่ฮั่น มณฑลหูเป่ย ประเทศจีน ตั้งแต่วันที่ 30 ธันวาคม 2562 ซึ่งต่อมาได้พบผู้ป่วยในหลายประเทศทั่วโลก จนกระทั่งวันที่ 11 มีนาคม 2563 องค์การอนามัยโลก (WHO) ได้ประกาศให้ COVID-19 เป็นการระบาดใหญ่ (Pandemic) ส่วนสถานการณ์ในประเทศไทยช่วงต้นปี 2564 มีรายงานพบผู้ติดเชื้อเพิ่มขึ้นเป็นจำนวนมาก ซึ่งโรงพยาบาลนพรัตนราชธานีพบการแพร่ระบาดในแผนกอายุรกรรมหญิงและชาย รวม 3 ราย จึงได้ทำการสอบสวนตามหลักการสอบสวนทางระบาดวิทยา (Outbreak investigation) 10 ขั้นตอน มีการประเมินความเสี่ยง และควบคุมโรคในรูปแบบ Bubble and Seal โดยมีการกักตัวบุคลากรกลุ่มเสี่ยงสูง 101 ราย ประกอบด้วย พยาบาล (ร้อยละ 58.4) ผู้ช่วยเหลือพยาบาล (ร้อยละ 14.9) แพทย์ (ร้อยละ 11.9) ผู้ช่วยเหลือคนไข้ (ร้อยละ 7.9) แม่บ้าน (ร้อยละ 5.9) และเภสัชกร (ร้อยละ 1) ตามลำดับ เมื่อทำการกักตัวบุคลากรเสี่ยงสูงในรูปแบบ Bubble and Seal โดยกักตัวและปฏิบัติงานในพื้นที่ที่พบผู้ป่วยและบุคลากรเสี่ยงสูง ร่วมกับการเฝ้าระวังการติดเชื้อและเฝ้าระวังอาการไข้ ไอ มีน้ำมูก เจ็บคอ เมื่อครบการกักตัว พบว่าบุคลากรกลุ่มเสี่ยงสูงและผู้ป่วยในหน่วยงานไม่มีการติดเชื้อ COVID-19 เพิ่มขึ้น รูปแบบการกักตัวแบบ Bubble and Seal จึงช่วยลดปัญหาเรื่องการขาดอัตรากำลังบุคลากรในการปฏิบัติได้เป็นอย่างดี เพราะไม่ต้องมีการหยุดงานในการกักตัวของบุคลากร ส่งผลให้โรงพยาบาลนพรัตนราชธานีไม่เคยมีประกาศหยุดให้บริการในแผนกใดๆ ดังนั้นจึงสามารถนำมาตรการนี้ไปปรับใช้ให้เหมาะสมกับสถานการณ์การกักตัวบุคลากรเพื่อลดการแพร่กระจายเชื้อได้อย่างมีประสิทธิภาพ และนำไปบริหารอัตรากำลังบุคลากรได้เป็นอย่างดี ทำให้โรงพยาบาลสามารถดำเนินกิจการได้อย่างต่อเนื่อง
