การสอบสวนโรคไข้หวัดใหญ่ในโรงเรียนการศึกษาพิเศษแห่งหนึ่ง อำเภอเมือง จังหวัดแพร่ ปี พ.ศ 2561

วันที่ 9 สิงหาคม 2561 กองระบาดวิทยาได้รับแจ้งจากสำนักงานควบคุมโรคที่ 1 พบกลุ่มก้อนผู้ป่วยอาการคล้ายไข้หวัดใหญ่ในโรงเรียนการศึกษาพิเศษแห่งหนึ่งในจังหวัดแพร่ จำนวน 18 ราย ทีมสอบสวนโรคดำเนินการสอบสวนโรคระหว่างวันที่ 14-17 สิงหาคม 2561 มีวัตถุประสงค์เพื่อยืนยัน ค้นหาสาเหตุ และควบคุมการระบาดของโรค โดยศึกษาแบบ retrospective cohort ค้นหาผู้ป่วยด้วยนิยามผู้ป่วยสงสัยคือผู้ที่มีอาการอย่างน้อย 2 อาการดังนี้คือ ไข้ ไอและเจ็บคอ ระหว่างวันที่ 14 กรกฎาคม ถึง 17 สิงหาคม 2561 และเก็บตัวอย่างโดยป้ายจากลำคอหรือหลังโพรงจมูกเพื่อส่งตรวจยืนยันไข้หวัดใหญ่โดยวิธี rRT-PCR ผู้ป่วยยืนยันคือผู้ที่มีผลตรวจยืนยันเชื้อไข้หวัดใหญ่ สำรวจสิ่งแวดล้อม และวิเคราะห์หาปัจจัยเสี่ยงที่เกี่ยวข้องกับการติดเชื้อไข้หวัดใหญ่ วิเคราะห์ด้วย univariate analysis และ multivariate analysis โดยคำนวณค่า risk ratio และ 95% confidence interval จากการคัดกรอง 317 คน พบผู้ป่วยสงสัย 89 รายและผู้ป่วยยืนยัน 13 ราย อัตราป่วย คิดเป็นร้อยละ 29.0 ผู้ป่วยยืนยันทุกรายพบเชื้อ Influenza A H1N1 2009 การระบาดครั้งนี้ไม่สามารถระบุต้นตอของการระบาด แต่การระบาดเกิดขึ้นหลังจากเปิดเรียนหลังปิดเข้าพรรษา ล้างมือเป็นปัจจัยป้องกันในกลุ่มนักเรียน (RR=0.22, 95% CI=0.04-0.84) และการใช้เสื้อผ้าร่วมกับผู้อื่นเป็นปัจจัยเสี่ยงในกลุ่มเจ้าหน้าที่ (RR=2.14, 95% CI=1.12-4.06) ผู้ป่วยทุกรายได้รับ Oseltamivir และปิดโรงเรียน 1 สัปดาห์เพื่อหยุดการแพร่กระจายเชื้อ ทั้งนี้นักเรียนทุกคนในโรงเรียนนี้เป็นกลุ่มเสี่ยงจึงควรได้รับวัคซีนไข้หวัดใหญ่
