การสอบสวนการระบาดของโรคอุจจาระร่วงเฉียบพลันในที่พักชั่วคราวจังหวัดกรุงเทพมหานคร ตุลาคม 2563

วันที่ 15 ตุลาคม 2563 เวลา 10.00 น. ศูนย์บริการสาธารณสุข 58 ล้อม – พิมเสน ฟักอุดม สำนักอนามัย กรุงเทพฯ ได้รับแจ้งจากหัวหน้าทีมบุคลากรข้าราชการที่มาปฏิบัติหน้าที่และพักอาศัยชั่วคราวในระหว่างรอปฏิบัติงานอยู่ในพื้นที่แขวงบางปะกอก เขตราษฎร์บูรณะ กรุงเทพฯ ว่าพบบุคลากรหลายรายมีอาการอุจจาระร่วงเฉียบพลัน ทีมเฝ้าระวังสอบสวนเคลื่อนที่เร็ว (Surveillance and Rapid Response Team: SRRT) ของศูนย์บริการสาธารณสุข 58 ล้อม–พิมเสน ฟักอุดม ร่วมกับหน่วยงานที่เกี่ยวข้อง ดำเนินการสอบสวนและควบคุมโรค ตั้งแต่วันที่ 15 - 30 ตุลาคม 2563 เพื่อยืนยันการวินิจฉัยและการระบาดของโรค ค้นหาสาเหตุและปัจจัยเสี่ยงของการระบาด พร้อมดำเนินมาตรการควบคุมป้องกันการระบาดของโรค ทีมจึงดำเนินการศึกษาระบาดวิทยาเชิงพรรณาโดยสัมภาษณ์ผู้เข้าข่ายการสอบสวนโรค เพื่อค้นหาสาเหตุและปัจจัยเสี่ยงของการระบาด การศึกษาด้านสิ่งแวดล้อม ร่วมกับการศึกษาทางห้องปฏิบัติการเพื่อตรวจหาเชื้อก่อโรคและใช้สถิติเชิงพรรณนา อธิบายข้อมูลทั่วไปของประชากรตามจำนวน ร้อยละ ค่าเฉลี่ย และค่ามัธยฐาน ผลการศึกษาพบการระบาดของโรคอุจจาระร่วงเฉียบพลันของบุคลากรที่มาปฏิบัติหน้าที่และพักอาศัยในที่พักชั่วคราวระหว่างรอปฏิบัติงานในจังหวัดกรุงเทพฯ ตั้งแต่วันที่ 15 - 30 ตุลาคม 2563 โดยพบผู้ป่วยทั้งสิ้น 71 ราย จากจำนวนบุคลากรทั้งสิ้น 480 ราย (อัตราป่วยร้อยละ 14.8) จำแนกเป็น บุคลากรระดับหัวหน้างาน 7 ราย บุคลากรระดับผู้ปฏิบัติงาน 64 ราย ค่ามัธยฐานของอายุเท่ากับ 42.0 ปี (20.0 – 53.0 ปี) ปัจจัยเสี่ยงที่พบ คือ การรับประทานอาหารที่ไม่ได้ปรุงสุกใหม่และปัจจัยด้านสุขาภิบาลอาหารของร้านประกอบอาหาร ผลการศึกษาทางห้องปฏิบัติการพบสารพันธุกรรมของเชื้อแบคทีเรียกลุ่ม enteropathogenic bacteria (Aeromonas veronii biovar sobria, Aeromonas spp., Plesiomonas shigelloides, Salmonella serovar Soerenga, Vibrio cholerae nonO1/nonO139/nonO141) ในอุจจาระของผู้ป่วย ร่วมกับพบเชื้อแบคทีเรียก่อโรคในระบบทางเดินอาหารกลุ่ม coliforms จากการเก็บตัวอย่างจากร้านประกอบอาหาร 2 ร้านและพบเชื้อ Bacillus cereus จากตัวอย่างอาหารที่เหลือจากมื้อก่อน ดังนั้น ร้านประกอบอาหารต้องดำเนินมาตรการด้านสุขาภิบาลอาหารโดยเคร่งครัด ทีมงานที่ดูแลด้านการจัดเตรียมอาหารควรกำหนดแนวทางกำกับดูแลกระบวนการนำส่งอาหาร เพื่อให้บุคลากรได้รับประทานอาหารที่ปรุงสุกใหม่ ร่วมกับกำหนดให้มีมาตรการสำหรับบุคลากรที่ต้องการเก็บรักษาอาหารที่รับประทานไม่หมดในแต่ละมื้อ รวมถึงแนวทางปฏิบัติก่อนการรับประทานอาหารที่เก็บไว้ในที่พักชั่วคราวเพื่อป้องกันการเกิดโรคในระบบทางเดินอาหาร
