ความคิดเห็นของบุคลากรทางการแพทย์ของไทยต่อการฉีดวัคซีนไข้หวัดใหญ่: ข้อมูลเชิงลึกจากการสนทนากลุ่ม

ความเป็นมา: ความลังเลในการฉีดวัคซีนในหมู่บุคลากรทางการแพทย์เป็นความท้าทายที่สำคัญในการป้องกันโรคที่ป้องกันได้ด้วยวัคซีน เช่น โรคไข้หวัดใหญ่ บุคลากรทางการแพทย์ที่ลังเลเรื่องวัคซีนมักไม่แนะนำการฉีดวัคซีนแก่ผู้ป่วย การศึกษานี้มีวัตถุประสงค์เพื่อทำความเข้าใจถึงความลังเลในการรับวัคซีนไข้หวัดใหญ่ของบุคลากรทางการแพทย์รวมทั้งศึกษาถึงการรับรู้และความกังวลเกี่ยวกับวัคซีนและโรคไข้หวัดใหญ่ที่มีผลกับความลังเลใจในการรับวัคซีน
วิธีการศึกษา: เป็นการศึกษาเชิงคุณภาพ ใช้การสนทนากลุ่ม ใช้แนวคำถามแบบกึ่งโครงสร้างในบุคลากรทางการแพทย์ 72 คนจากโรงพยาบาลจังหวัด 4 แห่งที่คัดเลือกโดยใช้การสุ่มตัวอย่างแบบเจาะจงจากภาคเหนือ ภาคใต้ ภาคตะวันออกเฉียงเหนือ และภาคกลางของประเทศไทยในเดือนสิงหาคม 2562 มีการใช้เทปบันทึกเสียง บันทึกเสียงภาคสนาม และการสังเกต นำมาใช้วิเคราะห์เพื่อสรุปประเด็นสำคัญที่เกี่ยวข้องกับกรอบงานความลังเลใจเกี่ยวกับวัคซีน เช่น ความเชื่อมั่น ความพึงพอใจ และความสะดวก
ผลการศึกษา: จากการสนทนากลุ่มในกลุ่มบุคลากรทางการแพทย์ 72 คน ครอบคลุมงานทุกประเภทในโรงพยาบาล วัฒนธรรมองค์กร บริบททางสังคม และบรรทัดฐานที่หลากหลาย ผลการวิจัยพบว่า มีความลังเลในการฉีดวัคซีนในโรงพยาบาลทุกแห่งในกลุ่มบุคลากรทางการแพทย์ทุกประเภท ปัจจัยสำคัญของความลังเลใจในการรับวัคซีน คือ ความกลัวว่าจะเกิดเหตุการณ์ไม่พึงประสงค์จากวัคซีน แม้ว่าบุคลากรทางการแพทย์จะมีการแสดงความมั่นใจในการฉีดวัคซีนสูงแต่ก็มีความรู้สึกไม่แน่นอนอย่างมากถึงความตรงกันของสายพันธุ์ไรวัสที่เป็นองค์ประกอบของวัคซีนและสายพันธุ์ที่ก่อโรคในขณะนั้น ขาดการสื่อสารเกี่ยวกับคุณประโยชน์และความเสี่ยงของวัคซีนในบุคลากรที่ไม่ได้ให้บริการผู้ป่วย ซึ่งมีความคล้ายคลึงกับความต้องการหลักฐานทางวิทยาศาสตร์ที่สูงในกลุ่มที่ให้บริการผู้ป่วยโดยตรง สิ่งที่น่าสนใจคือความเป็นวิชาชีพด้านสุขภาพแสดงออกอย่างสูงในบุคลากรทางการแพทย์บางคน ที่สนับสนุนให้บุคลากรทางการแพทย์ยอมรับวัคซีนถือว่าเป็นความรับผิดชอบในการปกป้องผู้ป่วยและคนอื่นๆ ประเด็นสำคัญอื่นๆ ที่เกิดขึ้น ได้แก่ ความกลัวเข็มฉีดยา ข้อจำกัดด้านเวลา และการจัดหาวัคซีนไม่เพียงพอ
สรุป: ข้อมูลทางวิทยาศาสตร์ที่ครอบคลุมประเด็นเกี่ยวกับวัคซีนและไข้หวัดใหญ่ควรได้รับการสื่อสารอย่างมีกลยุทธ์ไปยังบุคลากรทางการแพทย์ เพื่อเพิ่มความมั่นใจในประสิทธิผลของวัคซีน และเข้าใจถึงความเสี่ยงและประโยชน์ของวัคซีน การส่งเสริมและเพิ่มขีดความสามารถของบุคลากรทางการแพทย์ให้ตระหนักถึงหน้าที่ทางวิชาชีพในการปกป้องผู้ป่วยจากไข้หวัดใหญ่และบรรเทาการแพร่กระจายของไข้หวัดใหญ่จากบุคลากรทางการแพทย์อาจช่วยลดความลังเลในการฉีดวัคซีนในบุคลากรทางการแพทย์ได้
