Removal of Broken Instrument and Management of External Inflammatory Root Resorption in Mandibular Second Molar: A Case Report

Broken root canal instrument is one of the most troublesome incidents in root canal treatment. The broken instrument may hinder cleaning and shaping procedures with potential impact on prognosis of treatment.
External inflammatory resorption occurs as a result of chronic infection of necrotic root canals. When root canal infection persists for a long time, bacterial endotoxins and their products penetrate dentinal tubules inducing inflammatory resorption within the root and adjacent bone. Thus, the complexities of the tooth with open apex as a result of external inflammatory resorption is to prevent root canal fillings from extrusion beyond the root apex. This case report describes dental examination, diagnosis, treatment plan and management of broken instrument and external inflammatory root resorption.
After 3 months follow-up examination, patient reported regression of clinical signs and symptoms. Follow-up radiograph found decreased size of periapical lesions.
