Fifty participants were recruited by random sampling from patients receiving medical care at a diabetes mellitus and hypertension clinic, Nakhon Phanom Hospital. Participants were randomly assigned to experimental group (n=25) or control group (n=25). Data were collected from March to May 2021. Instruments for data collection were a chronic kidney disease stage 3 perception questionnaire (CKD-3PQ), and a health behavior questionnaire. The tool for conducting study was the illness representation based education program; which consisted of a teaching material set, a handbook, and self-monitoring record form. Content validity was verified by five experts and obtained reliability of .86. Data were analyzed using descriptive statistics and inferential statistics.</span></p><p><span class="fontstyle0">The results revealed that after the experiment, the experimental group had a statistically significantly higher mean score of illness perception, mean score of health behaviors, and mean of estimated glomerular filtration than those of the control group (p&lt;.05). In addition, mean of systolic blood pressure, diastolic blood pressure, and microalbuminuria of the experimental group was statistically significantly lower than those of the control group (p&lt;.05).</span></p><p><span class="fontstyle0">This study indicated that the illness representation based education program can improve illness perception, health behaviors, and clinical indicators in patients with stage 3 chronic kidney disease with a rapid decline of kidney function.</span></p> ER -
