The participants consisted of 64 diabetic patients with poor glycemic control and were equally divided into experimental group or control group. The experimental group received a self-management program based on Kanfer’s concept and the control group received usual care. Data were analyzed using descriptive statistics, chi-square test, paired t-test and independent t-test. The results showed that after receiving the program, the experimental group had knowledge and self-management scores higher than before receiving the program and higher than the control group, with the statistically significant level of p&lt;0.05. Also, after receiving the program, the experimental group had HbA1c levels less than before receiving the program and less than the control group, with the statistically significant level of p&lt;0.05.&nbsp;</p> ER -
