Early screening and investigating the risk of kidney degeneration is, therefore, important for patients as they can be treated promptly. Perceived risk and severity of the disease make the patients more likely to avoid the threats that help to slow down kidney degeneration. The nursing innovation &nbsp;was &nbsp;developed from 26 empirical evidences. According to the evidence-based practice. The &nbsp;tool was a rating scale composed of 7 risk factors &nbsp;predicting the progression of the disease to the end stage.The scores of each factor were 1- 3 points, the total scores were translated into 5 levels. The results showed that the patients’s risk perception was higher after the use of innovation (t = 14.14, mean difference of 4.4, 95% CI 3.75 to 5.05, p&lt;0.001)</p> ER -
