TY - JOUR AU - ศิเศรษฐกุล, สุรชัย AU - กงตาล, อรสา PY - 2017/07/04 Y2 - 2024/03/28 TI - Development of care and screening service for dementia at risk elders at chronic care clinic, Mueang Nat health promotion hospital, Nakhon-Ratchasima Province, Thailand JF - Journal of Nursing Science and Health JA - JNSH VL - 40 IS - 3 SE - Research Article DO - UR - https://he01.tci-thaijo.org/index.php/nah/article/view/108278 SP - 72-82 AB - <p>บทคัดย่อ</p><p>การวิจัยเชิงปฏิบัติการนี้มีวัตถุประสงคเพื่อพัฒนาบริการคัดกรองและจัดการดูแลผูสูงอายุสมองเสื่อมที่ คลินิกโรคเรื้อรังระดับปฐมภูมิ เลือกกลุมตัวอยางแบบเจาะจง ไดแก ผูสูงอายุโรคเรื้อรังจํานวน 320 คน ผูดูแล แกนนําชุมชน 70 คน และทีมสุขภาพ 13 คน เก็บขอมูลโดยสัมภาษณเชิงลึกและสังเกตกอน-หลังเขารวมกิจกรรม พัฒนาสมอง จํานวน 52 คน วิเคราะหขอมูลเชิงเนื้อหาและใชสถิติ Chi-Square และ Paired t-test ผลวิจัยพบวา ทีมบริการมีความรู ทักษะเพิ่มขึ้น พัฒนาบริการคัดกรองสมองเสื่อมที่คลินิกโรคเรื้อรังและในชุมชนไดครอบคลุม ผูสูงอายุ 3 กลุม คือ (1)กลุมปกติ (2)กลุมเสี่ยง โดยจัดกิจกรรมพัฒนาศักยภาพสมอง 6 ครั้ง ผูสูงอายุมีคาคะแนน เฉลี่ยเพิ่มขึ้นทุกดาน (ทั้งความสนใจ ความจํา และการรับรู) อยางมีนัยสําคัญทางสถิติ (P ≤ 0.000) (3) กลุมผูปวย ไดรับการสงตอ มีการดูแลตอเนื่องระยะยาว (Long Term Care) ครบวงจร และมีเครือขายชุมชนรวมสนับสนุน พัฒนาบริการ</p><p>คําสําคัญ: การพัฒนาบริการคลินิกโรคเรื้อรังระดับปฐมภูมิ, การคัดกรองภาวะสมองเสื่อม การจัดการดูแล ผูสูงอายุสมองเสื่อมในชุมชน</p><p>Abstract This mutual action research focused on development of dementia screening and care management for elders with dementia at risk at chronic care clinic and in community under primary care unit. Participants consisted of 320 chronically ill elders, 70 caregivers and community leaders, and 13 health teams. Data was collected through in-depth interviews and observations. Content analysis and chi-squared and paired t-test were performed for data analysis. 320 Elders in chronic care clinic were screened and 52 dementia at risk were cared for divided into 3 groups: normal, high-risk, and dementia. 6 rounds of brain stimulating&nbsp;activities were performed. The research found that health care team and caregivers obtained more knowledge and improved skills toward dementia screening and care. The activity results showed that the mean scores of attention, delayed recall, and orientation were statistically significant higher than before intervention (p ≤ 0.000). Elders with dementia were appropriately referred and continuously care by community-based long term care team.
