@article{เติมสินทรัพย์_ตรีเดช_ปั้นดี_2013, place={ิBangkok}, title={ประสิทธิผลการดำเนินงานการตรวจคัดกรองความเสี่ยงโรคเบาหวานและ โรคความดันโลหิตสูงของหน่วยบริการปฐมภูมิ กรุงเทพมหานคร The Effectiveness of Diabetes Mellitus and Hypertension Screening in Primary Care Units in Bangkok}, volume={29}, url={https://he01.tci-thaijo.org/index.php/bcnbangkok/article/view/28923}, abstractNote={<p align="center">บทคัดย่อ</p><p>                การวิจัยนี้มีวัตถุประสงค์เพื่อประเมินประสิทธิผลการดำเนินงานการตรวจคัดกรองความเสี่ยงโรคเบาหวานและโรคความดันโลหิตสูง ของหน่วยบริการปฐมภูมิ กรุงเทพมหานคร โดยเก็บข้อมูลของหน่วยบริการปฐมภูมิ กรุงเทพมหานคร เขตพื้นที่ 13 กรุงเทพมหานคร  จำนวน 148 แห่ง กลุ่มตัวอย่างเป็นพยาบาลหรือเจ้าหน้าที่สาธารณสุข เก็บข้อมูลโดยใช้แบบสอบถาม ระหว่างเดือนเมษายนถึงเดือนสิงหาคม 2555 ได้รับแบบสอบถามคืน จำนวน 135 ฉบับ (ร้อยละ 91.2) วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าทดสอบ Independent sample t-test กำหนดระดับนัยสำคัญทางสถิติ α =0.05</p><p>                ผลการวิจัยพบว่า  ประสิทธิผลการดำเนินงานการตรวจคัดกรองความเสี่ยงโรคเบาหวานและโรคความดันโลหิตสูงของหน่วยบริการปฐมภูมิ กรุงเทพมหานคร ส่วนใหญ่อยู่ในระดับปรับปรุง ร้อยละ 72.6 และกระบวนการบริหาร ทรัพยากรการบริหารและปัจจัยจูงใจ ส่วนใหญ่อยู่ในระดับดี ร้อยละ 97.0, 77.8 และ 97.8 ตามลำดับ และพบว่าการประชาสัมพันธ์เพื่อให้ผู้มารับบริการเข้ารับตรวจคัดกรองโรคเบาหวานและโรคความดันโลหิตสูง และการเปรียบเทียบผลการดำเนินงานกับเป้าหมายที่กำหนดไว้ ส่วนใหญ่มีการปฏิบัติบางครั้ง ร้อยละ 25.9 และ 32.6 ตามลำดับ เมื่อเปรียบเทียบความแตกต่างของกระบวนการบริหาร ทรัพยากรการบริหารและปัจจัยจูงใจ ระหว่างหน่วยบริการปฐมภูมิ กรุงเทพมหานครที่มีประสิทธิผลการดำเนินงานการตรวจคัดกรองความเสี่ยงโรคเบาหวานและโรคความดันโลหิตสูงของกลุ่มที่มีการดำเนินการระดับดี กับกลุ่มที่มีผลการดำเนินการระดับปรับปรุงพบว่า แตกต่างกันด้านความเพียงพอของเจ้าหน้าที่ การตรวจสอบความถูกต้องของจำนวนเงินที่ได้รับจัดสรร และลักษณะงาน อย่างมีนัยสำคัญทางสถิติ  ข้อเสนอแนะจากผลการวิจัย คือ  หน่วยบริการปฐมภูมิบางแห่งควรมีการให้บริการเชิงรุกควบคู่ไปกับการให้บริการในหน่วยบริการปฐมภูมิโดยมีการสนับสนุนด้านทรัพยากรการบริหาร ได้แก่ ปัจจัยด้านบุคลากร ปัจจัยด้านงบประมาณ และปัจจัยด้านวัสดุ อุปกรณ์ให้เพียงพอต่อการดำเนินงาน รวมทั้งควรมีการประชาสัมพันธ์อย่างต่อเนื่องให้ประชาชนรับทราบถึงนโยบายด้านส่งเสริมสุขภาพและป้องกันโรคอย่างชัดเจน และผู้บริหารควรมีการกล่าวชมเชยและให้รางวัลแก่ผู้ที่ปฏิบัติงานดีเพื่อเป็นขวัญและกำลังใจให้ผู้ปฏิบัติงาน</p><p align="center">ABSTRACT</p><p>                 The objective of this study was to evaluate the effectiveness of screening programs to evaluate the risk of diabetes mellitus and hypertension among the general public at 148 Primary Care Units (PCUs) in the 13th Bangkok Area. The sample was nurses or public health technician. The information was collected during April – August 2555 using questionnaires as the tool. There were 135 returned questionnaires (91.2%). The statistics used for analysis were percentage, mean, standard deviation, and Independent sample t-test with statistical significance at α=0.05.</p><p>                The study found that the majority of screening programs for diabetes mellitus and hypertension in Primary Care Units in the Bangkok Area were at the level of “needs to be improved” (72.6%). Most had management processes, resources for management, and motivation factors at the “good level” (97.0%, 77.8% and 97.8%, respectively). The launch of a public campaign to encourage the target group to come for the screening program and comparison of past performance with the target group had occasionally been done in some PCUs. (25.9%  and 32.6%, respectively). Upon comparing the differences in management processes, resources for management, and motivational factors between the Primary Care Units which had previously performed screening programs at the good level and In PCUs with programs that needed to be improved, it was found that there were differences in adequacy of number of personnel, validation of allocated budget, and the characteristics of task descriptions with statistical significance. </p><p>                Recommendations:  there should be preventive and promotional health services along with curative services with the support of sufficient management resources, e.g. personnel, finance, and medical supplies.
