Effects of Development Discharge Planning Programme of Chronic Obstructive Pulmonary Disease Patient in Phrae Hospital

Abstract

The purposes of this one group, quasi-experimental research was to study the effects of development discharge planning program of chronic obstructive pulmonary disease (COPD) patients at Phrae Hospital. The objective compared 1) nursing service behavior pre and post programme. 2) the patient's knowledge of the discharge planning program between the group who receiveddischarge planning program by nurse with the group without programme. The sample were simple randomed and included 30 nurses and 60COPD patients from medical department at Phrae Hospital. The research instrument was the discharge planning development programmethat consisted of COACH model, dialogue and after-action review for nurses.The four steps of COACH model included 1) clarifying needs 2) objective setting 3) action plan designing 4) checking activities, theprogramme took place over12 weeks and the questionnaires nursing service behavior and the knowledge discharge planning programme for COPD patients. Thecontent validitywas 0.6-1.0 andcronbach's alpha coefficient were 0.72, 0.73. Descriptive statistic, paired sample t-tests and independent sample t-tests were used for data analysis. The results of study indicated that mean score of nursing service behavior were significantly greater (p < 0.01) after implementation of the program than before and the patient's knowledge of the discharge planning programme after received discharge planning program by nurse in programme were significantly greater (p < 0.01) than nurse without programme
