By co-operation from multidisciplinary health care team, Village health&nbsp;volunteers (VHVs) and DM patient’s representatives, this R&amp;D focused on participation all&nbsp;stakeholders at all processes. The 3 consecutive stages of operating procedure consisted of&nbsp;1) DM care models study before development at Ranong Hospital by using qualitative and&nbsp;quantitative research methods. 2) Creating an integrated DM service system model using&nbsp;qualitative research method. 3) Tried out and evaluated of this integrated service system&nbsp;model by using quasi-experimental research methods.<br>&nbsp; &nbsp; &nbsp;This DM care system development expanded to the community by the multidisciplinary&nbsp;health care team. Family Practice nurse received the capacity building, DM practice&nbsp;guidelines was developed for public health network in Ranong District. The proactive service&nbsp;system was also covered DM drug management system including the drug dispense from&nbsp;Ranong hospital, health information system, DM care service at health promotion hospital of sub-districts and home visit by multidisciplinary health care team from provincial hospital&nbsp;with families participation and network partners in the community.<br>&nbsp; &nbsp; &nbsp;After the implementation of this service system, the percentage of DM patients&nbsp;for blood sugar levels control was improved from 12.98% in 2017 to 26.60% in 2018. DM&nbsp;patients satisfied with the health care services for rapid accessibility, convenience, reducing&nbsp;the waiting time and getting closely care by health care team.<br>&nbsp; &nbsp; &nbsp;The development of the diabetes service system by an integrated multidisciplinary&nbsp;public health care team with community involvement can be an efficient model to provide&nbsp;the continuous care for DM patients.</p> ER -
