Utilization of Orem's self care theory for nursing care of childhood patients with diabetic ketoacidosis: case study

Diabetic ketoacidosis (DKA) is a critical and emergency complication in pediatric patients with type 1 diabetes mellitus (DM) and resulting in a high rate of death. In this study, two cases of pediatric diabetic ketoacidosis patients were reported. There are two female pediatric patients, 8 and 12 years old, who have been physical stressed from infection leading to absolute insulin deficiency. Common symptoms such as frequent urination, low blood glucose level, shortness of breath or rapid and deep breathing were causes of hyperglycemia, metabolic acidosis, and ketosis. Patients receive nursing care based on Orem's Self-Care Deficit Nursing Theoretical Framework to improving patients' health outcomes through nurses' contributions. The nursing practices, adequate fluids, insulin, and oxygen, antibiotics and maintaining the balancing of blood sugar levels were provided in the emergency and critical condition. The results of nursing practices shown that blood glucose level returned to conventional levels, improves blood circulation and breathing and high patient recovers. The distribute patient plan was conducted by teaching patients and patient’s parent to take care of themselves about eating, exercising, insulin injection, blood glucose levels surveillance and infection prevention. Patients are making an appointment for a follow-up examination in 1 week.

Nursing care for patients with cerebral aneurysm: case study

Cerebral aneurysm is a critical condition that causes high mortality and disability, resulting in less blood supply to the brain, leading to cerebral ischemia. From the case study of nursing care for a patient undergoing surgery for cerebral aneurysms in this case, it was found that patients with aneurysms needed to be diagnosed quickly and accurately. Close monitoring and evaluation of patients using the nursing process are necessary, as well as the correct assessment of neurological symptoms and providing coordination with multidisciplinary teams to ensure patients are safely protected from life-threatening conditions or those causing disability. The nursing care should be evidence-based and includes the reported changes of neurological crisis and vital signs, assessment of signs and symptoms of cerebral insufficiency and increased intracranial pressure, monitoring fluid and electrolyte imbalances, and patient’s rehabilitation for good quality of life. The family is ready to take care of disabled person after discharge from the hospital. This is an important role of nurses.

ขอกล่าวคำว่า สวัสดีกับท่านผู้อ่านทุกท่านครับ กระผม ผู้ช่วยศาสตราจารย์ ดร.ทันตแพทย์ยสนันท์ จันทรเวคิน อาจารย์ประจำคณะทันตแพทยศาสตร์ มหาวิทยาลัยกรุงเทพธนบุรี ได้รับมอบหมายจากท่านอธิการบดีให้ทำหน้าที่บรรณาธิการ วารสารการแพทย์โลกาภิว้ตน์ ตั้งแต่วันที่ 27 เมษายน พ.ศ. 2566 ตามนโยบายของท่านอธิการบดีที่จะรวมวารสารสำหรับเผยแพร่ผลงานวิชาการของคณาจารย์ บุคลากร และนักศึกษาของคณะทางวิทยาศาสตร์สุขภาพ มหาวิทยาลัยกรุงเทพธนบุรี ให้เป็นวารสารฉบับเดียว

ดังที่เราทราบกันว่า ปัจจุบันสังคมไทยและอีกหลายประเทศทั่วโลกกำลังเข้าสู่สังคมผู้สูงอายุ (aging society) ข้อมูลจาก Economic and Social Commission for Asia and the Pacific (ESCAP) ขององค์การสหประชาชาติ แสดงให้เห็นว่าเมื่อสิ้นปี ค.ศ. 2022 จะมีประชากรสูงอายุ (มากกว่า 60 ปี) ประมาณ 670 ล้านคน อาศัยอยู่ในภูมิภาคเอเชียแปซิฟิก หรือคิดเป็นประมาณ 1 ใน 7 ของจำนวนประชากรทั้งหมดในภูมิภาคนี้ และคาดว่าสัดส่วนจะเพิ่มเป็น 1 ใน 4 ในปี ค.ศ. 2050 (1) โดยแนวโน้มดังกล่าวก็พบในประเทศไทย และส่งผลกระทบในด้านต่าง ๆ ทั้งสังคมศาสตร์ เศรษฐศาสตร์ รวมทั้งด้านวิทยาศาสตร์สุขภาพ

เมื่อประชากรมีอายุยืนยาวขึ้น ย่อมเลี่ยงไม่ได้ที่จะพบปัญหาสุขภาพเพิ่มมากขึ้น ในมุมวิชาชีพของกระผมที่ทำงานด้านสุขภาพช่องปากก็ให้ความสำคัญเรื่องนี้เป็นอย่างมากเช่นเดียวกัน ในการประชุมแผนยุทธศาสตร์ของราชวิทยาลัยทันตแพทย์แห่งประเทศไทยเมื่อช่วงต้นปีที่ผ่านมา ได้มีการบรรจุโครงการจัดตั้งสาขาทันตกรรมผู้สูงอายุ ขึ้นเป็นสาขาเฉพาะทางด้านทันตแพทยศาสตร์ มีการฝึกอบรมและสอบอย่างเป็นระบบ ซึ่งแสดงให้เห็นถึงความสำคัญของงานด้านนี้เป็นอย่างมาก หรือในหลักสูตรหลังปริญญาของมหาวิทยาลัยของเรา กรณีของสาขาวิชาทันตกรรมรากเทียม เราคงทราบอยู่แล้วว่ามุ่งให้การรักษาทดแทนฟันที่สูญเสียไป ซึ่งมักพบในกลุ่มวัยกลางคนถึงวัยสูงอายุ แต่ในกรณีสาขาวิชาทันตกรรมจัดฟัน ซึ่งพวกเราคงนึกถึงการรักษาในกลุ่มเด็กและวัยรุ่น ก็ยังมีหัวข้อการให้การรักษาทางทันตกรรมจัดฟันในผู้ใหญ่ (adult orthodontics) รวมทั้งมีการพัฒนาเทคนิคทางศัลยกรรมที่ใช้ร่วม เพื่อให้การรักษาผู้ป่วยกลุ่มนี้ (2)

อย่างไรก็ตาม การดูแลประชากรกลุ่มนี้ จำเป็นต้องบูรณาการองค์ความรู้จากหลากหลายสาขา ทางวารสารได้รับเกียรติจากศาสตราจารย์ ดร.ชัยยงค์ พรหมวงศ์ นักวิชาการด้านศึกษาศาสตร์ที่มีชื่อเสียง และท่านเคยดำรงตำแหน่งรองอธิการบดีฝ่ายวิชาการ มหาวิทยาลัยกรุงเทพธนบุรี ได้กรุณาเขียนบทความเรื่อง “วิทยาการปัจฉิมวัย: นวัตกรรมการจัดการศึกษา สาธารณสุข และสวัสดิการสำหรับสังคมสูงวัย” เพื่อแลกเปลี่ยนความรู้และประสบการณ์ ซึ่งจากบทความดังกล่าว ท่านผู้อ่านจะเห็นความเชื่อมโยงการจัดการศึกษาที่ไม่ได้มีเฉพาะการเรียนการสอนด้านวิทยาศาสตร์สุขภาพเท่านั้น แต่ต้องบูรณาการองค์ความรู้ ทั้งในแง่ศิลปะ นันทนาการ อาชีพ ธุรกิจ วิทยาศาสตร์และเทคโนโลยี ศึกษาศาสตร์ไปจนถึงวิชาศาสตร์แห่งชีวิต ซึ่งในความเห็นของกระผม มหาวิทยาลัยกรุงเทพธนบุรีของเรามีศักยภาพ สามารถจัดการเรียนการสอนแบบสหสาขารองรับหลักสูตรในลักษณะนี้ได้อย่างสมบูรณ์

นอกจากพันธกิจด้านการจัดการเรียนการสอนรองรับสังคมผู้สูงอายุที่ได้กล่าวมา มหาวิทยาลัยของเรายังมีความพร้อมทั้งในแง่การจัดบริการวิชาการให้ก้บประชากรกลุ่มนี้ รวมทั้งการทำงานวิจัยทั้งงานวิจัยชุมชน งานวิจัยคลินิก รวมทั้งงานวิทยาศาสตร์พื้นฐาน (basic science research) ซึ่งเราสามารถใช้ “วารสารการแพทย์โลกาภิวัตน์” ของเราเป็นสื่อกลางในการเผยแพร่องค์ความรู้เกี่ยวกับสังคมผู้สูงอายุทั้งระดับชาติและนานาชาติได้เป็นอย่างดีครับ

Ultimate Life Science: innovative management of education, public health, and welfare for aging societies

Critical problems facing Thailand during the past 30 years is the decreasing numbers of young and middle age population which the number of ultimate age population continuously increases. Thus, it deems necessary for Thailand to modify the paradigm in planning, preparing, providing services, and assessing the aging population to get ready for taking care of the members of Thai aging societies. This article is aimed to present a new concept in developing the innovative Ultimate Life Science (ULS) as a new branch of Health Sciences. This is a multi-disciplinary program of study involving 12 disciplines spanning from the fields of Health Sciences to Social Sciences and Humanities embracing the body of knowledge, nomenclature, modes of inquiries, and moral, ethical standards for members of the ULS profession to perform their professional and academic work efficiently in caring health and welfare of the aging members to transform their life from the present ones to the next world by setting the ideology (philosophy, missions, visions, and commitments), goals, objectives, and educational management for professional degree and non-degree programs in ULS through the process of identifying desirable attributes of the graduates, conducting situational analysis, developing experience-based instructional systems, providing ubiquitous educational technology, identifying appropriate ULS instructional approaches, producing ULS instructional packages, implementing ULS instruction, and conducting ULS evaluation.
