2026-10-012026-10-01364239242Successful treatment of methemoglobinemia in two boys with G6PD deficiency and favism following ascorbic acid therapy
https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/287752

Background: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a common X-linked hematological disorder that predisposes individuals to acute hemolysis under oxidative stress. A rare complication is the co-occurrence of favism and methemoglobinemia, a condition where hemoglobin is oxidized to the ferric (Fe3+) state, impairing oxygen transport. Management is complicated because methylene blue, the first-line treatment for methemoglobinemia, is contraindicated in G6PD deficiency.

Case Report: This report describes two Thai boys (aged 2.8 and 3.2 years) who presented with respiratory distress, low oxygen saturation (SpO2 83–89%), unresponsive to oxygen therapy, and dark urine following fava bean ingestion. Laboratory findings revealed acute intravascular hemolysis with presence of ghost and blister cells on peripheral blood smears, alongside elevated methemoglobin levels (3.3% and 8.0%). Both patients were newly diagnosed with G6PD deficiency, with case 1 confirmed as the G6PD Mahidol mutation (487G>A) and case 2 confirmed as the G6PD Viangchan mutation (871G>A). Due to the contraindication of methylene blue, both patients were treated with ascorbic acid (intravenous in case 1; oral in case 2) and blood transfusions. The oxygen saturation normalized within 2 and 5 hours after ascorbic acid administration, respectively.

Conclusion: The co-occurrence of favism and methemoglobinemia is rarely reported, with only 16 previous cases. These cases highlight the importance of recognizing methemoglobinemia in G6PD-deficient patients presenting with desaturation and demonstrate that ascorbic acid is a safe, effective alternative for the treatment of methemoglobinemia. However, careful consideration of ascorbic acid dosage is essential to avoid potential pro-oxidant effects.

Keywords: G6PD deficiency, methemoglobinemia, favism, ascorbic acid
Warittha SuphoSurapong LertthammakiatPriengrutai Prasopchoknapaporn

https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/286785

Aggressive systemic mastocytosis (ASM) is a rare clonal mast cell neoplasm characterized by extracutaneous mast cell infiltration leading to progressive organ dysfunction. Hepatic involvement is uncommon but clinically significant, as it may manifest with portal hypertension and refractory ascites, closely mimicking cryptogenic cirrhosis; thereby, contributing to diagnostic delay. In this report, we present a case of ASM with predominant hepatic involvement and associated cytopenias, underscoring the clinical, laboratory, and histopathologic features critical to establishing the diagnosis.

A 77-year-old Thai man presented with a 6-month history of progressive abdominal distension, early satiety, and significant unintentional weight loss. He initially received a diagnosis of cryptogenic cirrhosis with portal hypertension. Laboratory evaluation revealed normocytic anemia, thrombocytopenia, hyperglobulinemia, and isolated elevation of alkaline phosphatase with preserved hepatic synthetic function. Imaging studies demonstrated cirrhotic liver morphology, splenomegaly, generalized lymphadenopathy, and large-volume ascites. Bone marrow biopsy showed hypercellular marrow with spindle-shaped mast cell infiltration expressing CD117, mast cell tryptase, CD2, and CD30. Liver biopsy demonstrated sinusoidal infiltration by atypical mast cells with concordant immunophenotypic features. Serum tryptase was markedly elevated, and the KIT D816V mutation was detected, confirming the diagnosis of ASM. The patient developed refractory ascites requiring repeated therapeutic paracentesis and was subsequently treated with midostaurin, resulting in substantial clinical improvement, resolution of ascites, weight gain, and partial hematologic recovery.
Tatsanai SattayaraksaSuwannaporn YaophrukchaiJutawut ThaipanichMutita Surakijboworn

https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/291168

Abstract:

Introduction: Blood transfusion should not only provide the intended therapeutic benefit but also prioritize patient safety through accurate and reliable laboratory testing. Objective: This study aimed to evaluate the performance of analytical testing among blood bank laboratories participating in the External Quality Assessment program in Thailand. Materials and Methods: The Division of Proficiency Testing, Department of Medical Sciences, distributed proficiency testing materials to 1,242 member laboratories from both the public and private sectors in 2024 and 1,243 laboratories in 2025, through three testing rounds per year, totaling six rounds over the two-year study period. Results: For ABO blood group testing, more than 90% of laboratories achieved satisfactory performance in most rounds, except for rounds 3/2024 and 3/2025, with satisfactory performance rates of 72.3% and 77.9%, respectively. The test items distributed in these rounds were B3 and AB3 subgroups. For RhD testing, more than 98% of laboratories achieved satisfactory performance in most rounds, except for rounds 1/2024 and 2/2025, with satisfactory performance rates of 97.5% and 90.9%, respectively. The test items distributed in these rounds were RhD negative samples. For antibody screening and antibody identification, more than 97% of laboratories achieved satisfactory performance in most rounds, except for rounds 2/2024, 3/2024, and 1/2025, with satisfactory performance rates of 96.0%, 96.7%, and 96.5%, respectively. Conclusion: The findings indicate that proficiency testing materials involving complex or uncommon cases encountered in routine practice may increase the likelihood of incorrect test results. Continuous development of laboratory personnel’s knowledge and technical skills is therefore essential to improve the accuracy and reliability of test results and to support safe transfusion practices and optimal patient care.

Arida KlomkliangRuethaichanok PhenpooPipalin PromparditDhitiwass Suvagandha

https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/287638

Abstract:

Introduction: Management of patients on the waiting list for deceased donor kidney transplantation (DDKT) remains a major challenge in organ transplantation. Understanding the factors that influence access to transplantation is essential for optimizing allocation systems. Objective: This study aimed to evaluate the cumulative incidence of DDKT and identify independent predictors of transplant probability among patients with end-stage kidney disease (ESKD). Materials and Methods: This study analyzed a cohort of 5,276 ESKD patients registered at the Organ Donation Centre, Thai Red Cross Society (ODC-TRCS), during January 1, 2010, and December 31, 2020. Demographic characteristics, ABO blood groups, human leukocyte antigen (HLA) sensitization and HLA-A, -B, -DR haplotypes were analyzed. Transplantation was assessed by using cumulative incidence functions, with death treated as a competing risk. Significant factors were identified by using descriptive statistics and multivariate Fine–Gray sub-distribution hazard models. Results: A total of 1,996 patients (37.83%) underwent transplantation. The median follow-up time for kidney transplantation was 3.5 years. The probability of receiving a DDKT was 15% within the first year and increased to 45% by year 3. Older age was strongly associated with increased transplant likelihood, particularly among patients aged ≥60 years (subdistribution hazard ratio [SHR]: 5.63, 95% confidence interval [95% CI]: 4.12-7.70; p < 0.001). Intermediate (50-80% PRA) and high (81-100% PRA) sensitization were associated with modestly higher transplant probability compared with low sensitization (SHR 1.37; 95% CI 1.02–1.84; p = 0.037 and SHR 1.41, 95% CI 1.01-1.99; p = 0.045). Each additional year of dialysis reduced the likelihood of transplant by 29% (SHR 0.71, 95% CI 0.67-0.75; p < 0.001). ABO blood group and HLA haplotype were not significant. Conclusion: Age, HLA sensitization and dialysis duration significantly influence the probability of DDKT. These findings provide a framework for refining organ allocation policy and may serve as one consideration in its improvement.

Supasiri OunjaiNatkamol ChansatitpornPrasong KitidumrongsukSupanit NivatvongsOytip Nathalang

https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/287474

Background: Inherited hemolytic anemia (IHA) caused by red cell membrane and erythroid transcription factor defects remains underrecognized in Northeastern Thailand. We aimed to describe the clinical spectrum, molecular findings and transfusion outcomes of pediatric IHA at a regional tertiary care center. Methods: We conducted a prospective cohort study at Maharat Nakhon Ratchasima Hospital from 2023 to 2025. Twenty-nine children with clinically suspected inherited hemolytic anemia were enrolled. Clinical features, hematologic parameters, transfusion history and family studies were reviewed. Common SPTB variants, including Providence c.6055T>C, Buffalo c.6074T>G and Chiang Mai c.6224A>G, were tested using multiplex ARMS-PCR with high-resolution melting analysis. KLF1 testing was performed in selected cases. Results: SPTB variants were identified in 22 of 29 patients. Hereditary pyropoikilocytosis (HPP) was the most common phenotype and was associated with homozygous or compound heterozygous SPTB variants, particularly Buffalo-containing genotypes. Most patients with HPP had severe neonatal-onset anemia, transfusion dependency and frequent perinatal complications, including hydrops fetalis. Patients with hereditary elliptocytosis and heterozygous SPTB variants had mild, non-transfusion-dependent disease. Two patients had compound KLF1 variants and transfusion-dependent anemia, one of whom had confirmed neonatal-onset disease. Most affected families were from Nakhon Ratchasima Province. Conclusion: Recurrent SPTB variants, particularly the Buffalo variant, were frequently identified among children with suspected IHA referred to our center and were associated with severe HPP phenotypes. Early molecular diagnosis may facilitate accurate classification, counseling and timely management.
Punnavit Khongphithakskul

https://he01.tci-thaijo.org/index.php/JHematolTransfusMed/article/view/286242

Background: Diffuse large B-cell lymphoma (DLBCL) is prevalent in Thailand; however, real-world data identifying risk factors for mortality in local practice remain limited.

Objective: This study aimed to evaluate the one-year mortality rate and identify risk factors for early death among patients with DLBCL treated with chemotherapy.

Methods: This single-center retrospective study analyzed data from medical records of patients newly diagnosed with DLBCL between 2019 and 2025 at a provincial hospital in Thailand. Patients who received palliative or end-of-life care shortly after diagnosis were excluded. Univariate and multivariate logistic regression analyses were performed to identify independent predictors of mortality within the first year of treatment. Adjusted odds ratios (aORs) and 95% confidence intervals (CIs) were reported.

Results: Of 97 eligible patients (54% male; mean age ± standard deviation, 63.1 ± 12.2 years), the one-year mortality rate was 24%. Univariate analysis demonstrated significant associations between one-year mortality and age ≥60 years, poor performance status, Ann Arbor stage III-IV, anemia, chronic kidney disease stages 3-5, a low prognostic nutritional index, B-symptoms, serum albumin <3.0 g/dL, serum uric acid ≥6.8 mg/dL and an international prognostic index (IPI) ≥3. Multivariate analysis identified three independent predictors of mortality: high IPI (aOR, 11.80; 95% CI, 1.36-91.88; P=0.03), hypoalbuminemia (aOR, 5.92; 95% CI, 1.59-22.06; P=0.008) and hyperuricemia (aOR, 3.74; 95% CI 1.19-11.78; P=0.02).

Conclusions: The one-year mortality rate among patients with DLBCL in rural Thailand was 24%. In addition to the standard IPI, pretreatment hypoalbuminemia and hyperuricemia were identified as independent predictors of early mortality in DLBCL.
Naritsara CherdchooChatphatai MoonlaSunisa Kongkiatkamon
